RESUMO

The effects of three interventions designed to reduce sexual risk among Filipina female bar workers (FBWs) were compared with each other and with usual care (nonintervention). The interventions were developed iteratively by a community-based participatory research (CBPR) partnership comprising lay community members, organizational representatives (including nongovernmental organizations), and academic researchers from the United States and the Philippines. Peer educators and bar managers from 110 different establishments in three southern regions were recruited and trained to increase knowledge of HIV and of condom use rules and regulations within establishments, as well as to change attitudes about risk reduction, provide HIV and sexually transmitted infection (STI) testing referrals, and build condom use skills among FBWs. Compared with the control community, all three interventions increased HIV and STI testing; however, only FBWs in the combination peer-educator and manager-training intervention significantly increased condom use from baseline to 2-year follow-up. Condom use was significantly associated with higher HIV knowledge, attendance of a prevention class, and being taught how to use condoms properly. Given these findings, research is warranted to further explore and understand various forms of commercial sex work and to test adapted peer-educator and manger-training interventions within HIV epicenters.

RESUMO

Nearly 30 million people have died of AIDS during the past 27 years. More than 90% of people with HIV live in the developing world. The virus does not discriminate by age, race, gender, ethnicity, sexual orientation, or socioeconomic status-anyone is susceptible. HIV/AIDS has been particularly devastating on women and girls who now comprise more than 50% of people aged 15 years and older living with HIV. Successful strategies with significant impact on new HIV infections include strong governmental support, voluntary counseling and treatment, harm-reduction, community outreach, and universal access to treatment. This study presents the results of educational and social structural/environmental behavioral interventions among young women and their male partners. A 10-year study among high-risk populations (female bar workers and their male customers) in the Philippines identifies major components of a model HIV prevention program.

This study assesses the results of a 2-year community-based peer education program aimed at increasing HIV/AIDS knowledge, attitudes toward condoms, and condom use behavior among taxicab and tricycle drivers in the Philippines. Pretest, posttest, and follow-up data were collected throughout the educational intervention program. The results of the repeated measures analysis of variance (ANOVA) indicate a significant change on knowledge about HIV/AIDS from baseline to posttest and from posttest to follow-up (F=449.27, df=2, p<.001). There was also a significant change on attitudes about condom use from baseline to posttest and from posttest to follow-up (F=425.19, df=2, p=0.001), and a significant effect on condom use behavior with commercial sex workers from baseline to posttest and follow-up (F=428.31, df=2, p=.001). The peer-mediated intervention was found to be an effective means of HIV/AIDS prevention among taxi and tricycle drivers in the Philippines.

A 3-year, longitudinal, quasi-experimental study using participatory action research (PAR) was conducted to determine the feasibility and efficiency of an expanded sexually transmitted infection (STI) HIV/AIDS prevention program among diverse high-risk male heterosexual populations in the southern Philippines. A total of 3389 participants ( approximately 200 males from each of 18 study groups) were recruited, and 221 were trained as peer counselors to develop educational materials and reinforce safe sexual practices among their peers. Condom usage (36.10% to 38.70% to 46.31%), attitudes towards condoms (21.67% to 24.55% to 25.15%) and knowledge about HIV/STI transmission (41.87% to 42.19% to 33.31%) increased significantly from baseline to post-test and 6-month follow up, respectively (p < 0.01). Furthermore, the reported STI incidence decreased significantly (7.4% to 4.6% to 2.4%, respectively). Changes differed significantly between the intervention and control group at post-test and follow up (p < 0.01). These findings illustrate the appropriateness of using PAR methodology in promoting and sustaining positive behavior change.

The purpose of this research is to examine how condom use is affected by specific aspects of the work environment: (1) social-structural and environmental influences and constraints, (2) mandatory condom use policy, and (3) the level of social influence and reinforcement between manager and employee. A total of 1,340 bar workers and 308 nonestablishment freelance workers comprise the study group. In establishments where a condom use policy exists, female bar workers were 2.6 times more likely to consistently use condoms during sexual intercourse compared with establishments that do not have such a policy in place. The results suggest a need for the development of comprehensive educational policies in all entertainment establishments, including regular meetings with employees, reinforcing attendance at the Social Hygiene Clinic, promoting AIDS awareness, making condoms available in the workplace, and mandating 100% condom use behavior among all employees.
